Public & Private Employees must show identification cards to the Department an application form and the money will be paid after delivery of the completed form along with Syarat2 follows: -
1. Job Appointment Letters - Original
2. Confirmation Letter-Original Works
3. Employer Declaration - (Last place is still in charge)
